The potential winner will also be sent a relevant tax form W-9 (Request for Taxpayer Identification) and 1099-misc form from the Prize Provider to be eligible for the prize, or an alternate winner may be selected. Winner may be required to furnish proof of identity, address and birthdate in order to be verified as the winner to receive the prize.
